The Lifeform® Ultimate Executive Footrest is upholstered with temperature-sensitive LIFE-Foam™ pressure and temperature sensitive memory foam to reduce stress on tired feet.  The non-slip, “bean bag” underside moves with you to improve your circulation by comfortably adjusting your legs and feet to every angle.  Reduce fatigue by using this footrest with your LIFEFORM ergonomic office chair.
Please note Wood frame is only available in Black or Cherry finish.

LIFEFORM Limited Warranty guarantees the construction of all of our products to be free from defects in materials and workmanship for a period of 10 years from the date of purchase.


All of LIFEFORM’s Upholstery Coverings are protected by a ten (10) year limited warranty.
COM (Customer’s Own Material) is not covered under LIFEFORM’s warranty.


LIFEFORM foam products are of the industry’s highest quality. LIFE-Foam material (including visco-elastic and high density) will see a gradual decrease in viscosity (bounce back) over many years of use. This is a natural occurrence and is not covered by our warranty. LIFEFORM warrants to the original purchaser that LIFE-Foam used in LIFEFORM chairs will be replaced or repaired at our option, without cost, if deemed defective due to structural integrity or faulty workmanship within five (5) years from date of purchase.


  • This Warranty applies to Personal (Non-Commercial) Use only.
  • Limited Warranty applies to the original owner and is not transferable.
  • The Limited Warranty covers parts only. Labor and freight are not included.
  • Customers are responsible for transportation costs to and from the dealer or LIFEFORM.
  • Obvious signs of abuse, misuse or negligence voids this limited warranty.
  • LIFEFORM is not responsible for damage caused by transport, improper assembly or installation, maintenance, modification, tampering or for the damage or degradation of product that may occur due to extreme hot, cold or humid temperatures after it has left LIFEFORM.
  • Weight verification may be required to ascertain whether warranty is void.
  • Customer’s original bill of sale and serial number must be provided at the time of claim.
  • Limited Warranty begins from the date of purchase – if customer’s Original Bill of Sale is not provided then by default the date of manufacture will be used.
  • LIFEFORM does not Warrant:

    Normal wear and tear.

    Although LIFEFORM ensures the highest quality of workmanship in all of its products, LIFEFORM does not cover normal wear, fading, or damage that has occurred during the life of the upholstery.

    Marks, scars, wrinkles or stretching, which may occur in natural leather or inherently stretchy materials (such as UltraFabrics or Dream Weave).

    All leather pulls with use.  Puddling and stretching is generated by repeated use and is a hallmark of genuine leather and is not considered a defect.

    Matching of grains, colors or textures in Natural Leather.

    Exact color matching to cutting, samples, swatch cards or online/printed images.

CONTRAST PIPING:  You can add in an extra coordinating contrast piping, you may choose from any of the fabrics and leathers available.  *Extra $40

SIMULATED LEATHER (VINYL): Simulated leather (NaugaSoft) is excellent for those applications where fabric would be unsuitable because of exposure to moisture and soil, heavy traffic areas, or in medical applications where cleaning needs to be done more frequently. Our simulated leather fabrics are available in an extensive color range. *No Extra Charge



DREAM WEAVE: Dream Weave patterns are high performance 4-way stretch knit fabrics that allow extra comfort and movement to enhance the performance of our LIFE-Foam Memory Foam. Dream Weave fabric is knit to our specifications so there are few to no seams on the chairs as compared with traditional upholstery fabrics. Due to the unique knit specifications of Dream Weave fabrics they produce little no waste during production. Dream Weave products upholster beautifully and are suitable for residential, office, high traffic and contract seating. All patterns, with the exception of Meridian, are made with 100% Post-Consumer Recycled Polyester and are Certified Carbon Neutral. All Dream Weave fabrics are proudly made in the USA. *No Extra Charge



AMAZON / FOSTER / TERRAIN: Amazon, Foster and Terrain offer two-tone color combinations giving a subdued patterned effect. All Dream Weave patterns offer amazing stretch capabilities, which allow the user to experience the full effect of our LIFE-Foam Memory Foam.



DASH: Dash offers bright and cheerful colors in a unique texture design. Made entirely of 100% Post Consumer Recycled Polyester Dash, along with Amazon, Foster and Terrain, are also Certified Carbon Neutral. This means CO2 emissions from 100% of the electricity used by the manufacturer to produce this product have been measured and offset in accordance with The CarbonNeutral Protocol.



MERIDIAN: Meridian is the original Dream Weave pattern and offers the classic appeal of solid texture and color. Meridian is easy to maintain, requiring only the occasional vacuuming and dusting to keep it looking great.

All of our items have a FREE SHIPPING OPTION within the contiguous United States (we will need to quote out all Alaska, Hawaii, Puerto Rico and Canadian orders)

We strive to ship your items as fast as humanly possible--and try to regularly beat our goal of 5 business days from your order date.  We will send you tracking information within 24 hours of your order leaving the warehouse to the e-mail address you provided when checking out.  If you do not receive tracking information from us within six business days of your order, feel free to follow up with us at


All canceled orders are subject to your cc fees (close to 3%) which are non-refundable plus a $20 processing fee, whether or not your order has shipped. If your order has shipped, you (the buyer) will also be responsible for actual return shipping charges and any re-consignment fees associated. Refunds will only be issued to the original credit card that you used when placing your order.  


If you choose our free shipping option, you can expect to receive your item curbside within 7-10 business days of your item shipping--oftentimes sooner.  Our carriers will typically bring your chair to the front door for you--many of our chairs come in 2-3 boxes.  

  • Oftentimes, the chair arrives fully reclined in one big box with the armrests in another box and the footrest in the last box.  Typical assembly requires attaching the armrests and the footrest.  In some cases, a pedestal for your remote.  We have many installation videos available on our site and youtube channel.  If you do decide to go with curbside delivery, expect to have help with moving the chair into your home and desired location as many of our chairs are quite heavy.  
  • A dolly (or 2) will greatly assist in moving your chair to the right location in your home
  • Tip for moving your chair once it is assembled:  fully recline your chair; place dolly next to the chair and "tip" chair onto its side on the dolly.  You will then be able to roll your chair wherever you would like!  

If you choose white glove delivery, there will be an additional charge of $250 (even though we may spend more than $600 for this delivery option). Your item will be delivered to your home, placed in the room of your choice exactly where you want it and set up.  All debris will also be removed.  

  • Your chair will travel the bulk of its journey via LTL Freight and then transfer hands to the final mile/white glove delivery company.  Because we have to use an outside contractor for this service, your delivery time will be extended by one week. 
  • You will need to notify the White Glove company how many stairs are required for the delivery into the house and location of your choice when they call you to schedule your delivery.  If you have a difficult location (several stairs with challenging turns etc), please send us pictures so that we can make sure the delivery company is prepared.
  • If they arrive and it requires a 3rd man, there will be extra charges such as an extra return trip and fee--we don't want you (or us) to have any surprises.  Please really think about if you need white glove delivery--it's a lot more challenging and expensive to add it after your order has been placed.
  • All debris (boxes) will be removed with white glove delivery. If you end up returning your item or exchanging it for something else, it will also require a white glove pickup which can be quite costly.  Please keep this in mind as the delivery company is removing your debris.

We will do everything possible to ensure that your item arrives in the same condition that it left the manufacturer. Any product damage or loss is the responsibility of the shipping company, not Wish Rock Relaxation.BEFORE you sign the shipping weigh bill, please THOROUGHLY INSPECT YOUR ITEM. Signing for receipt of a damaged product removes the liability of the shipping company and Wish Rock Relaxation. Once you accept your shipment as non-damaged, the item is yours.

30 Day Return Policy

We want this to be one of the best purchases that you've ever made!  If you don't completely love your item, we will work with you to get into the right chair OR you can simply return it within 30 days of the ship date. This applies for all of our manufacturers except for UltraComfort. They now no longer accept any form of return or SWAP out due to buyer's remorse.  However, if your chair arrives damaged, they will replace it.

If you'd like to exchange your chair for another chair we carry, we would love to keep your business and will work to absorb all the fees to help you get into a better chair to meet your needs.

  • Item must be repackaged in it's original packaging, and must include all parts, hardware, controller and accessories.  If you no longer have packaging, you will need to supply or pay for repackaging and likely a white glove pickup. 
  • Applies only to orders within the continental US.  If we've shipped a chair to you in Hawaii, Alaska, Puerto Rico or abroad the shipping fees were likely $800-$1200 in one direction. 
  • If there are quality issues with our chair, we will work with you to get the chair repaired, replaced or exchanged for a different chair. If you refuse repair, replacement or even an upgrade, then you will be responsible for the shipping in both directions and any applicable restocking fees.

Restocking fees are also typical in the industry.  Specifically, several of our suppliers do charge them -- Human Touch and Cozzia (Svago, JP Medics, ErgoTec) are now 15% while Synca (Johnson) is 20%.

We will work with you to absorb the shipping and restocking fees if you choose to switch to another chair with us.  If you end up wanting to return your item, you will be responsible for the shipping in both directions and any applicable restocking fees.
